This is an example of poor communication. Designer told, "Layout a headline for 4 dead and 30 injured for our article on 3A to head the nation." And moves on to the other titles that are needed for the layout. Dave Bitton Lays out the story from the wire to fit the columns he is given. Both give items to the person doing the page layout. And then the editor gives his ok.
Under this scenario, the two people who did as they were told were the Layout designer and the writer. And the one who screwed up is the EDITOR. The other person who saw it, and gone "HOLD IT!" is the person laying out the page. And neither of these bums are being blamed for this!

Newspapers are not in the business of news...they are in the business of selling newspapers.
Thus the lack (intentional or not) of editorial oversight. It is an industry that is dying, and no longer enjoys the best and brightest waiting to break in to the business.
