My doctor had a question on his new paperwork asking if there were unsecured guns in my home and could children get to those guns. I didn't answer the question since it wasn't related to my health. I did answer the "Have you shared needles with a crack whore?" question.
Give your doctor this and insist he complete it and place it in your file.
