Wouldn't a solution to the problem be to simply (and respectfully you might not want to p.o. your Doc - talk about pissing in your chicken soup!) advise him/her that you do not consider that information to be their business? You don't need to be nasty about it; just be firm.
My Primary Care Doc knows I own guns; so does he. He also knows I consider him kith and would defend him as family.
Doctors - or anyone else - may ask whatever they wish. That does not constrain one to provide the requested information.
